The resources found here are specifically intended for those who are using the COVID-19 Population Health Navigator (PHN).
Given the rapid evolution of this tool in the West Region, these resources will be kept as up-to-date as possible with 'versions' and dates to indicated the latest updates.  The West Region Change Management Team from SW, WW, HNHB, and ESC areas will work together to maintain these resources for everyone.

What is the COVID-19 Population Health Navigator? 
The COVID -19 Population Health Navigator (PHN) Tool will help doctors navigage local residents through the important places of care in the right order in the fastest time. 

It will connect you with patients who need your help the most, and support you with processess and guidelines based on your patients' unique circumstances.  We acknowledge this is an extra step, but we believe harnessin the strengths of primary care to decant patients from assessment centres and emergency departments, while supplying important sector level data to public health and other parts of the healthcare system, are critical to continue flattening the curve.
