Description of Services:	Private home healthcare, specializing in medical and non-medical care for older adults and disabled adults needing additional assistance wherever their home is for them * in-facility relief staffing services * Develops and implements customized care plans Services include, but not limited to: personal care supported medication assistance and monitoring respite care hospital to home transition wellness checks with Personal Support Worker for medication checks and safety palliative care advocacy and coordination of medical and social services homemaking services including meal preparation, laundry, errands and light housekeeping services assistance with activities of daily living including accompaniment and transportation to local appointments for clients companionship range of Motion exercises Alzheimer's and related dementia care by "Certified Dementia Practitioner" Motor Vehicle Accident Care Programs Approved Registered Veterans Affairs, Canadian Forces, WSIB and insurance claims, Primer Accredited by Accreditation Canada.
